Obvious design mistake, because a small carbon composite ring on the rocker pushing against small steel bearing race is NOT an adequate fastening method for such a high load interface. Besides, the rocker has no bridge which could somewhat mitigate the effects of side-loading the rocker by more evenly spreading the load on both sides.
