Rear axle come loose while riding.. and no longer aligns

So on my giant trance advanced sx, the rear axle came loose while I was riding. Obviously I didnt notice until the drive train started acting weird, and then the chain came off. I looked down to see the axle hanging off the frame, almost completely out and at a weird angle. So I tried to thread it back in.

It got through the first hole, through the wheel, but the metal bit from the drivetrain, and the last hole in the other side of the frame, are out of alignment so when I threaded it into the the drive train bit it couldnt fit into the last hole where the screw is.

Which all sucks, but here's my questions. Why did the axle come loose? Is that a normal thing to happen? Thing is, it was only my third or fourth ride since getting it back from the bike shop after a repair, in which they took the wheel off and screwed it back on. Could it be their fault for not tightening it enough?

Had one over the bars crash too, but wasn't too gnarly. Maybe that did it, I dunno, doesnt seem quite right because it wasnt a super gnarly crash.

So yeah anyone have any thoughts? Also let me know if you know about damage like this, is it easy to repair or not.

Try putting the axle through the frame without the wheel in and see if you can get the axle to go though the second dropout (without any difficulty). If not then it sounds like you're frame may be misaligned. The other thing to check is that your wheel is sat in the dropouts properly when you try and fit the axle. Sounds silly but its easily done.

Post a few pics of you trying to install it, might make the problem more obvious to us.

With regards to you're other questions, axles, bolts and any other threaded components can work their way loose over time through vibration (unless they are lock-tighted in) so it is normal for things to loosen given time. Its not really the bike shop's fault as its you're responsibility to check axles and pivot bolts are tight before you ride, and even if they didn't tighten it fully you'll never prove it. That's not to say they did tighten it, which they should.

I'd be very surprised if the crash had caused it unless it caused heavy damage to one of the dropouts, have a look to be safe though.

I hope this helps, as I said post pics and I'm sure we can work out what's wrong Smile

I have the same issue with my bike. Rear axle wants to come unscrewed on advanced reign 1. It happens about once a month. I've never had a bike where a part consistently comes loose where I have to always be alert. The other thing is I'm constantly breaking the rear shifter cable at the derailleur, not sure if there is play in the system and the two are related. Usually there's play in the design plus vibration for something to come unscrewd. I noticed when doing downhill, lots of rock or vibration, it happens more frequently. I don't think it's normal, but I live with it.
