 It's a Davis & Sanford ATPG18. Got it for about $195 USD. It's really nice. SUPER smooth with the fluid head, and really sturdy. Also nicely adjustable.

 looks nice. but what is the advantage of having the dual levers

 Just better control. You can attach/detach both of them so you can run 2 handles, 0 handles, or 1 handle on either side.
