You've got the Rok Form iphone mount there, plus the Renthal Apex stem. Since I'm confident you know a stem when you see it, I'm guess you're talking about the iphone mount...? rokform.com. I got mine off theclymb.com for like $40. It was a couple years ago and they don't have any more in stock unfortunately, but it is a pretty fricken stout phone mount. It's got a 'cleat' which their phone case mounts to, plus a magnet on the mount and in the case, and their case comes with a lanyard that wraps around the mount so it stays in the area even if you have a big get-off.
