Crankbrothers Announce New Mallet & Stamp Trail Boa Shoes
@islandforlife: In my own experience, the Boa dial is really there to control the fit in the midfoot area, while the velcro strap is what actually keeps your foot inside the shoe. I appreciate this approach, since Boa shoes that rely only on the Boa for foot retention can leave you stranded on the trail if anything goes wrong.

Crankbrothers Announce New Mallet & Stamp Trail Boa Shoes
Many pedals let you swap to a different spindle length - in the case of Crankbrothers, the Mallet Enduro and Mallet Trail have a 57mm spindle stock, but you can drop to a 52mm spindle pretty easily. Might help offset it a bit. There are definitely some downsides with centered cleat boxes, including not being able to get out of the pedal at certain angles due to the shoe hitting the crankarm prior to achieving the necessary release angle. Best of luck with the knees!

Crankbrothers Releases New Stamp 1 Composite Flat Pedal
You can definitely service them - same bearings as Stamp 7. These just don't have a grease port.
