 i have that, u checked the chain is runnin on the bottom roller? kept coming off with me, you gotta tighten it up so much so it doesnt bounce of it.

 stiff links in your chain... buy a new chain, or lube it up well and fix the stiff links and then see if it still happens

 take off the chain tensioner get one that goes on the front sprokts youre chain will never ever fall off
